    The ceo showing up says: the hospital was in error and we're taking this very seriously. As a patient (or parent of patient) it also says: you're right to be upset, so are we and here's what we want to do to make sure it never happens again. Instead of: don't worry, your kid didn't even cry! (Read: your anger is not justified and it's no big deal)

          No not anecdotal. There is a TON of patient safety literature, it's a huge field. I covered it extensively in my MBA/MPH dual degree program. One of the best places to start would be the Institute for Healthcare Improvement or IHI. They offer courses that teach this material (I've taken many of them virtually).
