      My DH will be able to moonlight. I think it is pretty standard, but am not sure.

        I put "we're on the fence" b/c although he talks about this and that fellowship, it's so far away.... And I'm just not going to do the whole routine again- it's like choosing a residency specialty part 2...

        I think he probably won't do it. But that's just my best guess...

          I voted no and thank god.

          My Dh didn't do a fellowship and both of us are very happy about his decision. He gave the idea some serious consideration, but he knew that he did not want to go into academics and he wasn't in "love" with any particular ophtho speciality like glaucoma or cornea. Another factor that helped him decide against it was that in ophtho, unless you do a fellowship in either retina or occuloplastics (sp?), a fellowship does not equal more $$. For instance, in Dh's practice, one of the partners is fellowship trained in glaucoma and does the same thing as Dh. Additionally, Dh sees all of the peds ophtho cases in his practice and does strabismus surgeries, yet he's not fellowship trained in peds.

            Oh, totally. He's been talking about the Univ. of Toronto fellowship since the day he matched into NSG.

            Man, whatever. I am NOT moving to Canada for a year, and I am NOT schlepping a six-year-old over international boundaries every other week. If he wants to do this, fine. But better buy some postcards and stamps!

              Dh had his heart set on a toxicology fellowship from day one. The first year was major suckage; on home call 12x/month and ended up going in about 50% of the time. This second year is better. There are more fellows so he's only on call about 4x/month.

              He moonlights in the ER. In two ER shifts, he makes as much as he does in two full weeks as a fellow.

                Well DH is currently applying for a cardiology fellowship for 2009. The nice part about all of this is that we find out in July of 08 where we are going in July of 09 --nice to have a year to get life organized. He knew early on (as did I) that he wanted to specialize, so I didn't have a chance to talk him out of it . BUT, I did STRONGLY discourage doing a chief residency year. Personally, I don't understand WHY you would prolong residency if you didn't have to... Maybe if you're going into academics, or a really competitive fellowship...
                So, after residency, we have a 3 (or 4 if he subspecializes) years of fellowship. Ugh.
                Wife to a PGY-7 Interventional Cardiology Fellow, Mom to two. DS(7) and DD(3).

                  My husband started his nephrology fellowship this past July. It is a 2 year program, and my current state of mind is Yes, but I wonder how we'll get through it. Needless to say, it's been a hellish routine to get used to. I now just look forward to the months when he's on elective and can get home by 7PM instead of 10PM.

                  Having said all of this, in my moments of clarity, I do realize that this is what he wanted. He really is enjoying the fellowship and he's working hard so that in the end we can have a better life. What we have realized is that when we start to look for jobs next year, we need to look for one with flexibility. We're willing to slow his track down a little bit so that he can be home. I fully recognize that we might not find a job with the flexibility we're looking for, but for now, it's what's keeping me going.

                  As far as moonlighting is concerned, we originally thought he would have time to moonlight, but we've quickly put that idea to rest. No way with these hours. Thankfully we're in a position where it's not a necessity.

                    DH is finishing his interviews for a glaucoma fellowship on Friday. Depending on where we end up sort moves us between "it was o.k." and "I don't know how we will make it." We are tied to big cities or college towns because of my profession (which I have put on hold, sort of, while he is in these last stages so it will be my turn next). In a biger city with a more competitive market, the Glaucoma fellowship will make a difference in pay/marketability. I'm fine with the concept of fellowship. One more year won't hurt us...I am just really worried about a few of our possibilities...

                    Because it is fellowship, there are no rules about pay scale or work hours. Most programs are fine on hours, it is ophtho after all, but the pay issue can be a bit scary. O.K. down right terrifying.

                      The only thing that I will say in a public forum is that it has doubled, or more likely, tripled our earning potential. If cash is the exclusive basis for the decision, it's been a good choice. I have written other details in the private forums.

                        Luckily I didn't have to convince DH not to do a fellowship. His residency was so grueling that he just wanted to be done. He also didn't particularly need to specialize in anything and was happy to be a generalist. Plus the job market is very good right now and he wanted to ride the wave instead of specializing himself into a hole.

                          I put no and Thank God. For EM you need either a 4 yr residency program or a 3 yr program + 1 yr fellowship to do accademics (well that is the norm anyway). I told DH I wasn't moving for a year, (we too have a kiddo in school now, and at the end of this residency DD will be in kindergarten) so either take a four year program or make every effort to match in a place that has a fellowship. (you can at least high rank places that have a fellowship program you want). Luckily he doesn't like most EM fellowships. He still plays around with the idea of a sports medicine fellowhship one day if he gets burnt out in the ED. But for now I maintain my bummersticker "I want a normal life, so lets get there dude" A good friend on mine coined fellowship as the "F" word
