Psychiatric E/M Coding

Anyone out there have a cheat sheet for psychiatrists for E/M coding? We've got all of the forms from CMS but we're redoing our Electronic Medical Record to reflect the changes for January 1, 2013. Anyone have a simple form that they're using? CMS is getting rid of several of the codes that our docs use (so they're totally freaking out) and we're going to make this decision-tree that will be dummy-proof. (or doctor proof)
