I used to be a receptionist at a salon, and this is what I think you should do. Call the salon back and have your name and Pam's name specifically booked into those two appointments with those remaining two pedicurists.
Then tell Loud SIL that it turns out the pedicurist her appointment had been booked with won't be in that day and the salon had to cancel her appointment, but they're offering her a credit for a free pedicure in the future. Make it seem like you were each matched up with specific appointments all along, and the specific appointment that the salon cancelled was the one designated for Loud SIL. And then give her the freebie even though she's obnoxious--it'll be good karma for you.
I really think it's the most fair outcome for everyone even though it's annoying to see her get the free pedicure, and that this lie would qualify as a white lie.
As for looking ridiculous with three people watching two people get a pedicure, in my salon experience that would be a bit unusual, but if everyone is having fun and making an outing of it, I think the salon people will be laughing with you, not at you.
