Our son was 6 when DD came along - so a lot of things don't apply directly to A.
I really worked with C on self-sufficiency as soon as I found out I was pregnant. Things like brushing his own teeth. That way, it was an already established pattern and not the baby pushing him out of the way.
As for what to do with DS when I went into labor? We were lucky enough to have several neighbors on call. There was a couple across the street with a son the same age as DS. We did a trial run at a sleep over so that it wouldn't be a make or break experience when the time came.
We had to rely on the neighbors because my family was 3-1/2 hrs away and DH's family, while closer, wasn't able to help in that capacity (nor would I have asked it of them).
It was funny, because I had a bag packed for DS so all I needed to do was toss in his toothbrush. But, I didn't have squat backed for me, DH or the baby. My water broke while I was helping DS with his homework one evening so we lucked out not having to enact the middle of the night routine.
We had a second set of neighbors who's son was grown who would have come to our house if I had to go to the hospital in the middle of the night.
We notified my mom on the way to the hospital and she came the next morning. All in all it worked out fairly well.
