In short, be VERY sure you're up for all that is required when you sign on. Yes, that should always be the case, but this is one that I've heard of several people having very big second thoughts on, and it costs.

My husband got a "signing bonus" when he started, from the hospital. I think they may still do that, because it's hard to get family practice docs (at least good ones). It didn't pay it all, but a nice chunk of the loans. And we're in a fairly good size city (100,000) for Iowa, anyway.
