You've probably already gotten the gist of it from everyone else, but I'll throw my 2 cents in as well.
We did the one car thing for about 6 months this past year with DH as an intern and me working about 30 hrs/wk out of the house. Between the cost of the car itself, gas, and car insurance we actually saved quite a lot in those 6 months. Most days I would take him into work and go pick him up when he was done since I needed the car during the day (which was kind of a pain because that usually meant waking up at 4:30am, but it's only a 5 minute drive so I just came back home and went back to sleep), but it sounds like you wouldn't need it every day so you'd only have to do that occasionally. And as others said, I'm sure you could find another solution to the 3-4x you need to get to the airport. For your situation it sounds totally doable to me. Now that's assuming you guys don't live a ridiculously long way from his medical school.
